1.)    Application Form

2.)    Mental Health Form

3.)    $54.00 Money Order or Certified Bank Check, made payable to: "Division of State Police S.B.I."

4.)    $5.00 in cash for Firearms Identification Card. (if requested)

5.)    $2.00 in cash for Pistol Permit. (if requested) 

Either one or both can be requested on same application form.  Must be fingerprinted.

Applicants who had permits less than two years prior. 

1.)    Completed Application Form.

2.)    Mental Health Form.

3.)    Yellow record check form.

4.)    $15.00 Money Order or Certified Bank Check, made payable to: "Division of State Police S.B.I"

5.)    $2.00 in cash for each Pistol Permit requested.

6.)    $5.00 in cash for Firearms Identification Card if requested. 

Applicants who had permits over two years ago. 

1.)    Completed Application Form.

2.)    Mental Health Form.

3.)    $54.00 Money Order or Certified Bank Check, made payable to: "Division of State Police S.B.I."

4.)    $2.00 in cash for each Pistol Permit requested.  (if any)
