Recognition for a job well done is important.

Police Officers and civilian employees of the Middlesex Police are no exception.

If a Police Officer or other member of the Middlesex Police Department renders a service that you feel is worthy of a commendation, we would like to hear about it.

To commend a Middlesex Police employee you may:

	Telephone the Middlesex Police Patrol Division Commander by calling (732) 356-1900 ext. 229.
	Telephone the officer's supervisor directly
	Write to Chief James L. Benson of the Middlesex Police Department at:

1200 Mountain Ave.
Middlesex, NJ 08846

	Visit headquarters and speak to the desk officer.
	Complete the form below.

Please include as much of the following information as you can remember:

	Date, time, and location of the incident
	Name(s) and badge number(s) of the officer(s) involved
	Circumstances of the incident

The employee and his/her supervisor will be notified of the commendation.
